FAQ: Memberships

For additional general practice questions, please visit the main FAQ page.

For gastroenterology patients, you may only be looking for a one-time, in-depth digestive health evaluation. In this case, a single comprehensive visit may be enough to review your history, symptoms and prior testing to create a personalized care plan for you. For weight management patients, a single visit might optimize lifestyle habits (nutrition, activity, sleep, and stress), but if you are interested in medication therapy (which we strongly recommend for optimal weight loss and long-term weight maintenance), we encourage the memberships: ensuring optimal safety and effectiveness of the medication(s) requires observation over time.

If you are uncertain about committing to a membership, we are happy to discuss the options for you during your initial visit. If, for any reason, we are not a good fit for you, you are not obligated to sign up for a membership.

Working with our expert dietitian on GI or weight-related goals is a great opportunity to enhance your health journey. It is not required, as we recognize that you may already feel that your nutrition knowledge is quite strong or you may have an outside nutrition guide. In some cases, our doctors will recommend that you consider working with our dietitian, though any decision will be yours to make. All dietitian packs of visits are paid before the first visit, but the timing of your visits can be arranged according to your wishes.

We have designed our offerings to try to have an option for everyone. If you desire additional touchpoints, we recommend working with our dietitian and/or joining some of our online classes as a starting point. We are happy to discuss other alternatives for you on an individual basis.

We would be honored to work with you long-term, but we recognize that as a cash-based practice, this may not be possible for everyone. Once you reach your health goals, you have the option to transition your care and any prescriptions to another provider, such as your primary care physician or a general gastroenterologist (for GI conditions). If you elect to leave our practice for another provider, you are welcome back at any time.

For weight management: Our membership plans (above) that offer fewer annual check-ins are ideal for working on those last few health goals or after you’ve fully moved into weight maintenance mode. Additional check-ins with our dietitian may also be a great resource, as needed. If you return to our practice after some time away, we may recommend a more frequent visit cadence if you are shifting back to weight loss mode rather than remaining in the maintenance phase.

For gastroenterology: Once you reach your health goals, you are welcome to check in with us on an as-needed basis through our routine single follow-up visits (above).

We will not directly bill insurance for our care, but all of our services carry the potential to be reimbursed by insurance as out-of-network care. Labs and imaging are likely to be covered when the lab or imaging center is in-network for your insurance. Medication coverage and cost will be dictated by your plan’s formulary; medication cost may also vary by pharmacy (and whether it is in-network for your insurance). We ask you to verify all these details, as needed, before our initial visit.

At your request, we are happy to provide a bill that includes all services rendered. You can then take this bill to your commercial insurance provider. We regret that we cannot guarantee or predict a reimbursement level, which will depend on the specifics of your insurance policy. Government programs like Medicare and Medicaid will not reimburse out-of-network services.

For background information here: Traditionally, health insurance companies set the rules for provider reimbursement, which then dictates how much time we get to spend with you. By shifting to out-of-network care and keeping our patient panels smaller, our model allows us to spend more time with you at each visit, see you more frequently, and respond quickly to your questions or concerns through our inbox messaging.

Yes. Making lifestyle and behavioral changes deserves and requires one’s best efforts. If you are at a point where this might feel impossible, we would love to work through these dynamics with you, but some patients may choose to delay the start of their membership until these circumstances stabilize. We can discuss this during our intake visit and make our recommendations, but ultimately, the decision on when to begin is yours. Our sole focus is to ensure you get the most out of your membership!

Separately, some insurance policies require that a patient enroll in a weight management program for a period of time (often six months) before they will approve a prescription for one of the branded medications, especially semaglutide or tirzepatide. In this case, you might choose to meet with our dietitian or work on our lifestyle recommendations from the initial visit, before starting the membership at the same time that you begin medication to maximize its benefit. We are happy to approach this on a case-by-case basis to satisfy insurance requirements for medication approval.

We accept major credit cards, HSA / FSA cards backed by major brands, and direct deposits. All payments will be made through and processed by our electronic medical record, which prioritizes data security.

Memberships cannot be refunded during their term. After each membership period, your membership will not automatically renew. We sincerely hope all of our patients will choose to renew as long as you find our care beneficial, but there is no pressure to renew at the end of each term. It will be up to each patient to let us know of their desire to sign up for another membership or to renew on their own through our patient portal.

Any gastroenterology membership can be chosen at any time after your initial visit. Among the membership choices for weight management, we ask that you choose a 5-visit or 4-visit plan for the first year following your initial visit. After the first year, you are free to choose any frequency of visits for your second year of membership and beyond. Your physician or dietitian may give you their recommendation for a certain duration of membership or frequency of visits based on their work with you at the initial visit and/or during your current membership term.

Yes, we hope to reach as many patients as we can with the highest-quality information about the medical conditions we have studied in great depth. Classes are included in the cost of membership, but will incur an additional fee for those not enrolled in a membership. Please be advised that no information in these classes should be interpreted as medical advice. In order to receive formal medical advice and counsel, you will need to sign up for an initial visit, at minimum, with one of our providers.
